We are Medicaid providers for CCSP and SOURCE recipients who are elderly and those who are not elderly but physically disabled.  Care provided by our staff is geared towards keeping an individual living in the community and in their own home rather than being placed in a nursing home facility.  Our trained and competent staff provides the following services:

Skilled Nursing Services
  • medically related care that must be delivered by trained and competent licensed nurses.

Personal Support Service: 
  • assistance with activities of daily living (bathing, routine skin and hair care, mouth care, grooming and shampooing hair, assistance with dressing, assistance with toileting, back rubs, etc.) 
  • ambulation and transfer
  • assisting with scheduling medical appointments, arranging trips to the doctor, and accompanying to medical appointments
  • picking up prescription medications, reminding individuals to take medication, and assistance with self-administration of medication
  • light housekeeping (vacuuming, sweeping, doing laundry, ironing, etc.)
  • preparing meals & washing dishes
  • assisting with eating
  • grocery shopping
  • assisting with bill paying
 
Extended Personal Support Services:
  • the delivery of personal support services described above in the individual’s home which may also include relief of the person(s) who normally provides care and/or oversight (respite care to caregivers).
